INSTRUCTIONS FOR FILLING OF APPLICATION FORM

1.	No Column to be left blank (if not applicable should be marked as NA)
2.	Application Form should be submitted between 0800-1200 hours daily (except holidays) at the window of Visa-1 at the Consulate General of Pakistan, Dubai
3.	Application should be submitted by individual applying personally with all original documents.
4.	In case of children, both Parents (Mother & Father) need to given consent form/ NOC and both need to visit the window for submission of the forms. Consent form/ NOC be signed by the Parents at Visa Window. In case of single parent custody documents of the child are also required to be submitted alongwith the application.
5.	Visa will be refused on furnishing any false information.
6.	Signature wherever are required on application must be with blue ink.
7.	If applicant unable to visit the Consulate for interview after twice upon calling, his application will be rejected.

No Objection certificate from sponsor in UAE (Original)

Invitation from Pakistan on Affidavit (Notarized) in original alongwith copy of CNIC and valid mobile no of invitee.

Marriage certificate in case of spouse is Pakistani national (duly attested from Consulate General of Pakistan). Moreover marriage status must be updated in Nadra.

If applicant changed his nationality, will provide citizenship certificate.

If applicant is an employee he will attach NOC from company if he is in partnership so he will provide copy of Trade Licence.

Birth certificate copies of children must be attached with application if their parents are applying their visa.

11.

In case of Business Visa (following additional documents should also be attached):-

a.	Photo copy of trade licence in case of own business and NOC in case of employee.
b.	Personal bank statement of applicant for the last six months.
c.	Invitation letter from chamber of commerce (in original), if applicant wishes to visit more than one city, invitation letter for each city is required.
d.	Invitation letter from the company to which individual visiting on company printed letter head.
